Terms of Ministry

  1. I have read and agree to the Waiver of Liability and Waiver of Confidentiality.
  2. Fees are to be paid below in advance of the ministry session. I will be contacted to arrange a time mutually convenient.
  3. Rescheduling an appointment and cancellations require 24 hours notice.

Waiver of Liability

I acknowledges the following regarding Lapstone Ministries:
  1. As Christian Prayer Ministers, they not licensed counsellors, but are well trained and certified in Pastoral Prayer Ministry.
  2. They minister according to the principles in the Christian Bible.
  3. No guarantee can be or is made that I will receive any particular healing.
  4. All healing is under the direction of the Holy Spirit and is dependent on Him.

Waiver of Confidentiality

I relinquishes my right to confidentiality at the discretion of Lapstone Ministries, particularly for the following situations:
  1. Lapstone Ministries is required by law to report any suspicion of physical or sexual abuse that has been done or that is being done to a minor child. Situations of physical or sexual abuse of a wife or husband may also be reported at Lapstone Ministries' discretion.
  2. Lapstone Ministries is required to report any suspicion of potential or actual suicide attempts on the part of the Client.

Acknowledgment

I acknowledge the following by my request for an appointment:
  1. That all statements made are confidential, including all written information, and that legally and ethically these may not be disclosed without written consent except in the above specified situations.
  2. That I have read and accept all of the Limits of Liability, Limits of Confidentiality and terms of Prayer Ministry as described above.
  3. That Lapstone Ministries reserves the right to make such reports as ordered by law, whether or not they consult with the Client first.
